24 Hour On-Call Skilled Nurses We provide nursing when the patient needs it most. An RN is on-call 24 hours a day and can be reached by dialing 1-877-281-4000.
Pain Control Our major priority is patient care! Palliative care for pain control, comfort and symptom management is provided, as needed.
Personal Care/Homemakers/CHHA Homemaking services designed to give peace of mind and comfort.
Counseling & Bereavement Social Services provides counseling and makes appropriate community referrals. Spiritual services to provide direct counseling and assist with memorial preparations.
Volunteers Hospice volunteers compassionately give assistance comfort, and companionship through friendly visits, letter writing, reading, shopping, running errands, walking the dog or watering the plants.
Medications & Medical Equipment We provide medications, medical equipment, and supplies that are related to the terminal diagnosis, at no cost to the client.

Dietician A dietician is available to assess nutritional needs of the patient, provide education on the changes that occur as a result of the disease process, as well as implement a nutritional program which will assist in meeting the patient's nutritional needs. Therapists Assesses patient's physical or communication needs and works with the physician and Hospice nurses to coordinate services that will facilitate optimal patient comfort. |
